Navigating Surrogacy as a Plus-Size Woman: What to Know

If you’re a plus-size woman considering surrogacy, you may be wondering if your body type will affect your ability to become a surrogate. The short answer: not necessarily.

What is BMI, and Why Does It Matter in Surrogacy?

BMI, or Body Mass Index, is one of the metrics fertility clinics often use to assess a surrogate's eligibility. While BMI is not a perfect measure of health, many clinics use it as a screening tool to evaluate potential pregnancy risks.

Why Do Clinics Care About BMI?

It comes down to medical safety—for you and the baby. Most fertility clinics have a BMI cutoff for surrogates, typically between 30 and 35, with most preferring a BMI of 32 or less. Research shows that higher BMIs can be associated with certain pregnancy risks, such as:

  • Elevated risk of gestational diabetes or hypertension

  • Higher chance of C-section delivery

  • Potential complications during anesthesia

What if I’ve Had Healthy Pregnancies in the Past?

If you've carried your own children to term with minimal complications, that is a great start. Your surrogacy team and the clinic will take your previous pregnancy records into account when determining your eligibility. However, the qualifications for surrogacy are based on many factors and having "perfect" pregnancies of your own unfortunately does not guarantee you will qualify medically to become a surrogate.

Tips for Plus-Size Surrogate Candidates

If you’re worried your BMI might be a barrier, here are a few helpful tips:

  • Apply anyway: If your BMI is at or under 35, apply anyway. Many women are surprised to learn they meet the criteria or only need to make small adjustments to qualify.

  • Talk to your doctor: They can help you understand your personal health metrics beyond just BMI.

  • Don’t crash diet: Instead of drastic changes, focus on healthy lifestyle habits that support your overall well-being.
